Joining is a simple, two-step process! First, print the employee OR the contractor forms below, fill them out, and return them to the Cardinal Health Fitness Center. You may email the completed forms to GMB-DUB-FitnessCenter@cardinalhealth.com or bring them to the Fitness Center front desk. Once the paperwork is returned, you will receive login information for our membership portal, where you can edit your profile and schedule future fitness center appointments.
Upon your first arrival, the fitness center front desk staff member will hand you your check-in barcode that you will scan at the front desk at each visit, as well as go over the fitness center policies. Please note, that at this time, we require scheduled appointments in order to workout at the fitness center. If you have any issues scheduling your appointment, please email the GMB.
Lastly, we now have a mobile App, which makes scheduling appointments easier than ever and even has your check-in barcode saved! To download, you must have completed your enrollment forms. Information on the app can be found below.
